How three kinds of organizations run the cycle.

A laboratory, a physician practice, and a hospital outpatient department. Each one below is the path that kind of organization takes through G3 Flow, from the work that starts the claim to the payment that posts.

Laboratory

The order, the coverage, and the claim that follows

The work starts at the requisition. Coverage is checked before the specimen is billed, and a missing payer is a discovery step rather than a claim sent to the wrong plan. The claim is built from the order and scrubbed with the laboratory edits in that organization’s rule pack.

Once it leaves, acknowledgments come back as 999 and 277CA files and the claim stays on one matrix. When the ERA arrives, the deposit is matched, the payment posts, and the result writes back to the billing system. A denial opens as work, with an appeal draft attached to the claim it belongs to.

Physician practice

From the estimate at the desk to the statement

Eligibility runs before the visit. The benefits come back in plain language, and the estimate the front desk quotes is the one those benefits support. A payment taken at the desk is recorded with a receipt, on the same visit.

After the encounter, the claim is scrubbed, sent, and tracked. What the payer does not cover becomes a patient balance, and that balance goes out as a statement with a way to pay. A denial or a short pay is a piece of work, ranked with the rest of the insurance accounts receivable.

Hospital outpatient

Routing, authorization, and the month-end match

Coverage on file decides the route, including a secondary payer, so the claim is not sent to the plan that should have been second. A prior authorization is tracked on the service it covers, and the hospital’s own edits sit in a specialty rule pack that runs before the 837 is built.

Open insurance balances are ranked by age and amount. Remittances are matched to the deposits that actually arrived, posted once, and written back. Month-end is that match, across the outpatient departments that share the same billing system.

Your specialty decides the edits.

Rule packs cover laboratories, clinics, hospitals, behavioral health, therapy, and medical equipment.
